On this page you can download appropriate application forms and related information.
Our new forms for 2012 are designed for you to fill them in on your computer and then email them to us, or you can still print them, complete by hand and post back to us. If you email a form to us we recommend that you password protect the form to ensure that your data is kept secure. Please then provide the password to us separately.
Unfortunately we cannot provide our unable forms for download here (VIP or A/U) because we need to send a reply paid envelope and information for your doctor. Please contact Julie or Liz and they will send you all the necessary paperwork.
Please note that there are different application forms for chaplains, doctors, nurses and helpers. Please use the correct form and if you are in any doubt about the suitability of your medical condition, please contact the office for advice. A medical condition does not preclude you travelling as a carer, but it is important we understand everyone's capabilities so we can ensure that the helper team is balanced and able to manage all the care needs of the VIPs travelling. Whichever form you need, please also download and read our Privacy Policy. A medical handbook or group leaders handbook can be downloaded from the restricted area for those with password authorisation.
